please help - poem for darling wifes funeral

My 48 year old wife has just died and I am at my wits end. She had cancer so we had planned certain things about her funeral. I found a poem on the internet which mentioned flying and being on the next mountain and clouds but I cannot find it anywhere. I know this is a bit vague but can anyone help me ?
